It is your responsibility to complete and submit the form before attending the classes. You will be able to attend the class ONLY AFTER the form has been submitted.

Lessons cancelled by our teachers or Enact Music will be re-arranged for an alternative date, and students who are unable to attend the rescheduled class will be fully refunded for the missed lesson.
